Hi! My name is Meg Egan, and I am the School Nurse. My hours are from 9-2:30 every day during the school year. I will be here during the summer with the Baker Summer Discovery program from 11-2 each day from June 14th through August 6th. Medical Forms
ALL MEDICAL FORMS SHOULD BE FILLED OUT AND SENT IN BEFORE THE FIRST DAY OF THE SCHOOL YEAR.
For students new to Baker:
1) You will need a doctor’s physical and up to date immunizations: Certificate of Child Health Examination (PDF).
Parents please fill out and sign the Medical History portion of this form yourself.
For new Pre-Kindergarten students:

1) If your child has any medical or healthcare issues we should be aware of or updated on, please fill out the following form: Annual Health Services (PDF)
Other Medical Forms:
1) If your child has a severe allergy to food, bees, or other allergy that requires an Epipen, please fill out the following form with your doctor: Emergency Health Care Plan (PDF)
2) If your child has asthma, please fill out this form with your doctor.
3) To receive any over-the-counter or prescription medications at school (including any pain relievers), please fill out and have your doctor sign the Medication Authorization Form (PDF)
When Should a Child Stay Home
- contagious disease (let the school office know)
- unexplained rash
- fever (temperature of 100 degrees or above)
- red eye(s) with discharge and/or crusting on lashes and burning pain
- vomiting
- diarrhea
- obvious signs that your child cannot comfortably participate in class activities (even without a fever)
When Should a Child Return to School
- Once the child is fever-free for 24 hours, without the use of fever-reducing medication
- Once the child does not have recurrent diarrhea lasting more than 8 hours
- Once vomiting has subsided for approximately 24 hours, but if the child still does not feel well they should not come to school.
